The following schematic shows the design of a simple 5-band graphic equalizer PCB layout and circuit board diagram. This equalizer has five bands which cover all the audible frequencies, centered at 60, 250, 1,000, 4,000 and 16,000 Hz. The circuit consists of 6 IC’s, the well known LM741 op-amps five of which are used as active filters while the sixth is a mixer amplifier which returns the processed acoustic signal to the power amplifier.
Working voltage: 12V DC
Current: 50 mA
Frequency response: 20Hz-20KHz

How much? Where can I order from? Can I modify the center frequency to 800 hz, instead of 1K?,Top @ 7K-8K instead of 10K? Can I modify to operate off of 9v, instead of 12v?
